THE EVALUATION OF THE ALVEOLAR STRUCTURES AND SYMPHYSIS REGION OF THE SEVERE CLASS III AND CLASS I MALOCCLUSION

Objective: The aim of this study is to compare the mandibular alveolar and symphysis structures of severe adult Class III and adult Class I malocclusion. Material and Method: The pretreatment lateral cephalograms of 68 Class III adult patients who had undergoned orthognathic surgery and 74 Class I adult patients having minimum or moderate crowding were evaluated. Go-Gn-SN°, SNA°,SNB°, ANB°, Wits, gonial angle, molar and incisor alveolar heights, maxillary palatal width, mandibular incisor alveolar width (Id-Id'), symphysis width, height and symphysis ratio were measured. The width of the thinnest region of the symphysis at the level the B point (B-B') were also evaluated. Results: SNA°, ANB°, Wits appraisal, symphysis width and the thinnest width of the symphysis values (B-B') decreased in Class III group when compared to the Class I group (p<0.001). Vertical plane angle, gonial and lower gonial angles, SNB° and symphysis height increased in Class III group (p<0.001). Mandibulary alveolar width (Id-Id') was significantly decreased in the Class III group (p<0.001). Maxillary molar alveolar height was similar in both groups while upper and lower incisor alveolar heights and lower molar alveolar heights were found to be high in the Class III group (p<0.001). Conclusion: The symphysis height increased and symphysis width and mandibulary incisor alveolar width decreased in Class III group. It's important being careful about the decision of the compensation treatment in severe Class III patients. Similarly, decompensation of the mandibular incisors for preparing the orthognathic surgery should be considered for periodontal health.

Amaç: Bu çalışmanın amacı şiddetli erişkin Sınıf III ve erişkin Sınıf I vakalarda simfiz ve alveoler yapıları karşılaştırmaktır. Gereç ve Yöntem: Ortognatik cerrahi görmüş 68 erişkin Sınıf III bireyin ve moderate veya minimum çapraşıklığa sahip 74 erişkin Sınıf I bireyin başlangıç lateral sefalometrik filmleri değerlendirilmiştir. Go-Gn-SN°, SNA°,SNB°, ANB°, Wits, gonyal açı, molar ve keser alveoler yükseklikler, maksiller palatal genişlik, mandibular keser alveolar genişlik, simfiz genişliği, yüksekliği ve simfiz oranı ölçülmüştür. Ayrıca simfiz bölgesinin B noktası hizasında en ince yerinin genişliği (B-B') de değerlendirilmiştir. Bulgular: Sınıf III grupta SNA°, ANB°, Wits değeri, simfiz genişliği ve simfizin B noktasındaki genişliği (B-B'), Sınıf I grupla karşılaştırıldığında azalmıştır (p<0,001). Dik yön açısı, gonyal açılar, SNB° ve simfiz yüksekliği Sınıf III grupta artmıştır (p<0,001). Mandibuler alveoler genişlik (Id-Id') Sınıf III grupta önemli düzeyde azdır (p<0,001). Üst ve alt keser ve alt molar alveoler yükseklikler Sınıf III grupta artmış bulunurken (p<0,001), mandibuler molar alveoler yükseklik her iki grupta benzer bulunmuştur. Sonuç: Sınıf III hastalarda simfiz yüksekliği artmış ve simfiz genişliği ile mandibuler keser alveoler genişliği azalmıştır. Şiddetli Sınıf III hastalarda kompenzasyon tedavisi kararında dikkatli olunması önemlidir. Benzer şekilde ortognatik cerrahiye hazırlık için mandibuler keserlerin dekompanzasyonunda periodontal sağlık açısından dikkatli olunmalıdır.
