Lef ventricular dimensions and systolic function were investigated in twenty-Jour patients 2 to I fi years old (mean age: " 95- 5.28) with beta thalassemia were studied by two-dimentional. M-mode and color-Doppler echocardiography. In patients with beta thalassemia, left ventricular end diastolic diameter and posterior wall thickness were significantly increased (p 0.05). also lejl ventricular mass was increased, but diastolic thickness of interventricular septum was decreased (p 0.01). Left ventricular end diastolic volume, stroke volume, cardiac output and cardiac index were significanly increased in thalassemic patients (p • 0.01, p 0.05). These result suggests that in thalassemia patients left ventricular function may be impaired without any clinical sign and myocardial changes may begin in the pediatric age group. For initial assessment and follow up of thalassemic patients who are at extremely high risk for myocardial iron deposition echocardiography is a practical and noninvasive method. Ondokuz Mayıs Üniversitesi Tıp Fakültesi Çocuk Sağlığı ve Hastalıkları Pediatrik Hematoloji Bölümü'nde beta talasemi tanısıyla izlenen ve yaşları 2-IH yıl (ort. ".95-5.28) arasında değişen 24 hasta ile 24 sağlıklı çocukta 2 boyutlu. M-mode ve renkli Doppler ekokardiyagrafı yapılarak sol ventrikül boyut ve sistolik fonksiyonları incelendi. Beta talasemili grupta sol ventrikül diyastol sonu çapı ve arka duvar kalınlığının anlamlı derecede arttığı. inter\>entriküler septum diyastolik kalınlığının azaldığı ve sol ventrikül kitlesinin arttığı görüldü (p 0.05). Sol ventrikül diyastol sonu volüm , strok volüm, kardiyak output ve kardiyak indeksin aynı gru/>ta anlamlı derecede artmış olduğu saptandı(p 0.01. p 0.05). Çalışmamızın sonuçları beta talasemide çocukluk çağında bile. klinik bulgu olmadan bazı sol ventrikül fonksiyonlarında bozulma olabileceğini ve miyokardiyal değişimlerin başlayabileceğini göstermiştir. Miyokardiyal demir depolanma riski altındaki bu hastalarda kardiyak yapı ve fonksiyon değişimlerini erken dönemde saptama ve seri takipte ekokardiyografı uygun, güvenilir. ınvaziv olmayan bir yöntemdir. /Turgut Ozal Tıp Merkezi Dergisi l(l):39-46, 1994}
