Objective: The widespread use of intrauterine devices (IUD) has presented a variety of side and migration effects and complica¬tions. A common one is the dislocation of the device. We find it very important to stress the management of lost IUDs as it usual¬ly takes little effort to diagnose and remove migrated IUDs com¬pared to its possible, hazardous complications. Case Presentation: A 43 year-old menopausal woman presented to the emergency clinic of Haydarpaşa Numune Training and Research Hospital with severe lumbar pain. An IUD image was detected in the urinary system x-ray. The patient gave the history of an IUD inserted 15 years ago after her last delivery followed by numerous abortions and had been informed by the former physi¬cians that the IUD was possibly expelled. Not considered as an emergency case, she was sent to the Family Planning Unit for fur¬ther investigation. In her pelvic examination no IUD strings were detected. The IUD could't be imaged in the abdominal ultrasound examination so an abdominal computorized tomography (CT) scan was performed. The CT revealed the presence of the IUD in the abdominal cavity. After laparoscopic removal of the IUD, she was discharged with complete recovery. Conclusion: Plain abdominal x-rays which are economical and eas¬ily accessed are valuable in the detection of IUD's whenever their expulsions are uncertain.

Amaç: Rahim içi araçların (RİA) yaygın kullanımı sonucunda çeşitli yan etki ve komplikasyonlar ile karşılaşılabilmektedir. Bu problemlerden biri yer değiştiren RİA'lardır. İpi görünmeyen RİA'ların yönetiminde, göç etmiş RİA'ların neden olabileceği ağır komplikasyon-ları önlemek için yapılması gereken basit işlemleri vurgulamak amacıyla bu olgunun sunulmasını uygun gördük. Olgu: Postmenopozal dönemde olan 43 yaşındaki hasta , şiddetli bel ağrısı ile Haydarpaşa Numune Eğitim ve Araştırma Hastanesi Acil Polikliniğine başvurdu. Çekilen direkt üriner sistem grafisinde RİA'ın görüntülendi. Hastanın öyküsünde; 15 yıl önce kendisine RİA uygulandığını ancak uygulamadan sonra düşük ve küretaj nedeniyle kliniklere başvurduğunu ve kendisine RİA'ının düştüğünün söylendiğini ifade etmesi ve acil müdahele gerekmediğine karar verilmesi üzerine Aile Planlaması Ünitemize yön¬lendirilen hastanın spekulum muayenesinde RİA ipi gözleneme-di.Tüm batın ultrasonografisinde RİA'ın uterus boşluğu içerisinde görülmemesi üzerine, tüm batın tomografisi çekildi. Peritoneal kavite içinde RİA'ın görüntülenmesi üzerine laparoskopik girişim ile peritoneal kavite içinden RİA çıkarıldı. Hasta şifa ile taburcu edildi. Sonuç: Hastada kayıp RİA'dan şüphelenilen her durumda maliyeti son derece düşük olan ayakta direkt batın grafisi çektirmenin gerekliliğini vurgulamak istedik.
