Akut böbrek yetmezliği (ABY) saatler, günler ya da haftalar içerisinde gelişen, glomerül filtrasyon hızındaki (GFH) azalma sonucu böbreğin fonksiyonlarında görülen hızlı kaybın oluşturduğu bir klinik sendromdur. ABY, GFH'deki azalma ile tanımlansa da, tübüler ve/veya dolaşım sorunlarına bağlı olarak gelişebilir. Unutulmaması gereken önemli bir nokta da akut böbrek fonksiyon bozukluğunun birçok ol¬guda tek organ yetmezliği şeklinde değil, çoklu or¬gan fonksiyon bozukluğu ile birlikte bulunmasıdır. ABY'nin gelişme sıklığı, ülkemizde olsun, kayıt sistemlerinin geliştiği diğer ülkelerde olsun, iyi bilin¬memektedir. Tanımlama kriterleri uzmanlara göre farklılıklar göstermektedir. Sınıflama konusunda henüz ortaya konmuş bir fikir birliği yoktur. Bununla birlikte, hastanede yatan yoğun bakım hastalarının %20-25'inde (1), tüm hastaların ise %5'inde (2) ABY geliştiği düşünülmektedir. Hastanelerde yatan hasta¬larda görülen ABY olgularının çoğunda iskemi ve nefrotoksik ajanlara bağlı gelişen akut tübüler nekroz (ATN) önde gelen sorundur. ATN için başlıca klinik risk faktörleri; iskemi, hipotansiyon veya hi-poperfüzyon, ateş, sepsis sendromu ve nefrotoksik ajan kullanımıdır. ATN gelişen yoğun bakım hastalarının 2/3'ünde böbrek kanlanmasının azalması, sep¬sis ve nefrotoksik ajan kullanımı mevcuttur (1).
