Our Treatment Principles and Results in Pressure Sores

In this study, our experiences with 43 patients who were treated for pressure sore between the years 1993 and 2001 in the Department of Plastic and Reconstructive Surgery of Uludağ University Medical School were presented. Cases were investigated regarding the age, sex, etiologic factors, location of pressure sore and stage, preferred surgical methods and the complications of surgery. Forty-three cases had 70 pressure sores. Thirtythree cases were men and 10 cases were women. The mean age was 48 years. Most common etiologic factor was paraplegia with 34 cases and most common risk factor for paraplegia was traffic accident with 15 cases. Most common location was sacral area with 30 cases. According to the staging system, 61 cases were stage III and 9 cases were stage IV. Preferred surgical options were gluteal musculocutaneous flap for sacral pressure sores, tensor fascia lata musculocutaneous flap for trochanteric pressure sores and inferior gluteal musculocutaneous flap for ischial pressure sores. Most common acute postoperative complication was hematoma and seroma with 6 cases. Recurrence was observed in only 3 cases.

Bu çalışmada, 1993 ve 2001 yılları arasında, Uludağ Üniversitesi Tıp Fakültesi - Plastik ve Rekonstrüktif Cerrahi Anabilim Dalında tedavi edilmiş 43 bası yaralı olgu ile ilgili deneyimlerimiz sunulmaktadır. Olgular yaş, cinsiyet, etyolojik faktörler, bası yarasının anatomik lokalizasyonu ve evresi, tercih edilen cerrahi yöntemler ve karşılaşılan komplikasyonlar dikkate alınarak incelendi. Kırküç olguda toplam 70 bası yarası tespit edilmiş olup, olguların 33’ü erkek, 10'u bayan hasta idi. Olguların yaş ortalaması 48 idi. Bası yarası oluşmasında en sık karşılaşılan etyolojik faktör 34 olgu ile parapleji idi. Parapleji nedenleri arasında ilk sırayı trafik kazası 15 olgu ile almaktaydı. Olgularımızda bası yarası 30 olgu ile en sık sakral bölgede görüldü. Klinik evrelendirmelerine göre 61 bası yarası evre III ve 9 bası yarası evre IV olarak saptandı. İlk tedavi seçeneklerimiz, sakral bası yaralarında gluteus maksimus kas-deri flebi, trokanterik bası yaralarında tensor fasya lata kas-deri flebi ve iskiyal bası yaralarında ise inferior gluteus maksimus kas-deri flebi idi. Olgularımızda operayon sonrası en sık karşılaşılan erken komplikasyon 6 olgu ile hematom ve seroma oluşumu idi. Nüks sadece 3 olguda gözlendi.
