Introduction: Use of sutures for securing conjunctival autograft following pterygium excision has several drawbacks. We aimed to compare the use of 8-0 Vicryl sutures versus fibrin glue for fixating conjunctival autografts in patients undergoing pterygium surgery. Materials and Methods : A prospective interventional clinical study was carried out in a tertiary eye hospital. Sixty patients (60 eyes) with primary pterygium were randomised to undergo pterygium surgery using either 8-0 Vicryl sutures (30 eyes) or fibrin glue (30 eyes) to attach the conjunctival autograft. The patients were followed up for 12 months. Outcome measures were duration of surgery, complication, post-operative discomfort and recurrence of pterygium. Observations and Results : In the suture group, the mean operation time was 34.3 min (range 30-41 min) and in the fibrin glue group it was 13.13 min (range 11-17 min) {p <0.001}. Post-operative discomfort was significantly lower in the fibrin glue group than in the suture group (p <0.001). The incidence of complications were similar in both groups (4 each). At the end of follow-up, pterygium recurrence was observed in one eye (3.33%) in the fibrin glue group and in three eyes (10%) in the suture group (p <0.05). Conclusion : Application of fibrin glue instead of sutures for attaching the free conjunctival graft in pterygium surgery resulted in shorter surgical time, less post-operative discomfort and lower recurrence.
