52 perimenopausal women who has irreguler vaginal bleeding taken into this study in our hospital between 1997 and 1999. Histopathologic results of pipelle endometrial sampling versus probe curettage were: insufficient material 6% vs 6%, secretuar endometrium II.5% vs 13%, proliferative endometrium 21% vs 21%, incomplete secretuar endometrium 10% vs 10%, irregular proliferative endometrium 21% vs 21%,endometritis 11.5% vs I0%,simple endometrial hyperplasia] 3% vs 13%,atypical adenomatous hyperplasia 4% vs 4%, adenocarcinoma 2% vs 2%.In patients whose endometrial thicknesses are less than 8 mm, no endometrial pathology detected in both techniques. If we accept 8 mm thickness as a cut-off value, sensitivity is 100%, spesifity 22%, positive predictive value 26%, negative predictive value 100%. As a result: we concluded that in perimenopausal women who has irregular vaginal bleeding, we can detect the pathology by transvaginal ultrasonography correlated by either sampling techniques.

Jinekoloji polikliniğimize 1997-1999 tarihleri arasında başvuran 52 perimenopozal düzensiz vaginal kanaması olan hasta çalışma kapsamına alındı. Tüm hastaların endometrium kalınlıkları transvaginal ultrasonografi ile ölçüldükten sonra hastalara önce pipelle daha sonra probe küretaj uygulanarak endometrial örnekleme yapıldı. Pipelle ile yapılanlarda histopatolojik olarak; %6 yetersiz materyal, %II.5 sekretuar endometrium, %2I proliferatif endometrium, % 10 yetersiz sekretuar endometrium, %2I düzensiz pro lif e rat if endometrium, %I1.5 endometritis, %13 basit endometrial hiperplazi, %4 atipik adenomatöz hiperplazi, c/o2 adenokarsinom bulundu. Probe küretaj ile yapılanlarda ise %6 yetersiz materyal, %13 sekretuar endometrium, %21 proliferatif endometrium, % 10 yetersiz sekretuar endometrium, %2I düzensiz proliferatif endometrium, %I0 endometritis, %I3 basit endometrial hiperplazi, %4 atipik adenomatöz hiperplazi, %2 adenokarsinom bulundu. Transvaginal ultrasonografi ile endometrium kalınlıkları 8 mm 'den daha az ölçülen hastalarda her iki örnekleme yöntemi ile de endometrial patoloji saptanmadı. 8 mm eşik değer için sens i t iv ite %W0, spesifite %22, pozitif prediktif değer %26, negatif prediktif değer %J00 bulundu. Transvaginal ultrasonografi ile ölçülen endometrium kalınlığı, pipelle ile yapılan endometrial örneklemenin histopatolojik sonucuyla korele edilerek perimenopozal vaginal kanamalı kadınlarda etyolojik nedeni ortaya koyup tedavide davranış yönteminin belirlenebileceği sonucuna varıldı.
