This research to assess the mothers’ health care seeking behavior and associated factor on common childhood illnesses in Mekelle City 2012/2013. A community- based, cross sectional study was conducted in January, 10, 2013 to June 5, 2013. A total of 553 mothers who have under five were selected systematic random sampling method. Data were collected by interview and analyzed using SPSS for windows version 20 Descriptive, bivariate and multivariate logistic regression analyses were used to show frequency distributions and associations ATALAS 2012 was use to analyze the open ended question data. Based on the data collected from the 553 mothers who participated in the study 179(32.9 %) of them reported child sickness during a recall period of four weeks preceding the interview. Cough was the most common reported symptoms106 (18.9%), diarrhea, fever, eye problem, skin problem and tonsils (31(5.5%), 25(4.5), 10(1.8), 10(1.8), 2(0.4%) respectively). About 54.3% (n=100) of those mothers who reported child illness had visited modern health care service. Those who had not sought modern medicine constitutes 48(26.2%) used home remedy, 14(7.7%) self-administer medication, 6(3.3%) was give medication with the consultation to pharmacy,2(1.1%)took their child to holy water. Care seeking interventions have the potential to substantially reduce child mortality, in the country where common childhood illnesses are a major problem. However, in this study, care was sought in health care facilities by slightly half of the mothers with under five children during the sickness of their children. Most common symptoms reported in the area were fever, cough, diarrhoea, eye problem and skin infection. For the reported symptom of child sickness, most mothers had taken their children to health facility within 1-72 hrs.
