Outpatient antibiotic overuse of the university staff and their families

Abstract (2. Language):
Objective: This study aimed to observe the rate of outpatient
antibiotic use of the Mustafa Kemal University staff and their families,
the type of antibiotics used, under which diagnoses these
antibiotics were used and, preferences of different health care
units.
Methods: Health care records belonging to year 2004 of all aca¬
demic and non academic staff and their families were investigat¬
ed. Antibiotic prescriptions, diagnoses, and attending physicians
were determined.
Results: Of 11.163 outpatient visits, 4.613 (41.3%) had antibiotic
prescriptions. It was found that these prescriptions belonged to
1.942 different patients comprising faculty, officers and their fam¬
ily members. Approximately half of the antibiotics were prescribed
for children. Mean antibiotic consumption of the subjects were
2.4 ± 2.0 prescriptions annually and were significantly higher in
children (p<0.05). Twenty-seven subjects used more than ten
antibiotic cures per year. More than half of the antibiotics were
prescribed in the University Health Center, and the rest of them
were prescribed in university hospitals, state hospitals, primary
health care offices, and private practices/private hospitals, respec¬
tively. The most frequently indicated diagnosis was upper respiratory
infection. When other respiratory system diseases were
included, it was seen that 70% of the antibiotics were taken for
respiratory infections. Nine percent of the prescriptions were for
urinary tract infections. Penicillin-beta lactamase inhibitors, 2.
generation cephalosporins and macrolids were the most fre¬
quently chosen molecules, mostly in oral form. Antibiotic prefer¬
ences of the physicians by health units were significantly different.
Conclusions: The present study showed high rate of antibiotic
use among the staff and their families. Management of respirato¬
ry tract infections should be reconsidered in continuing medical
education, as three fourths of the antibiotics were used for respi¬
ratory infections.

Amaç: Üniversite çalışanlarının ve bakmakla yükümlü oldukları yakınlarının
ayaktan antibiyotik kullanma sıklığı, tercih edilen antibiyotikler,
bunların hangi tanılarla ve hangi sağlık kurumlarında yazıldığının
belirlenmesi amaçlandı.
Yöntem: Tüm çalışanların ve yakınlarının 2004 yılına ait tedavi kayıtları
incelenerek antibiyotik içeren reçeteler saptandı. Bu reçetelerdeki
tanılar, yazan hekimlerin çalıştığı kurumlar belirlendi.
Bulgular: incelenen 11.163 reçetenin 4.613'ünde (%41.3) antibiyotik
vardı. Antibiyotik reçeteleri öğretim elemanı, memur ve onların
yakınlarından oluşan 1.942 farklı kişiye aitti. Antibiyotiklerin
yaklaşık yarısı çocuklara yazılmıştı. Tüm grupta ortalama yıllık antibiyotik
kullanma sayısı 2.4±2.0 idi ve bu sayı çocuklarda daha
fazlaydı. Yirmi yedi kişi yılda 10 kez veya daha sık antibiyotik kullanmıştı.
Tedavilerin yarıdan fazlası kurum sağlık merkezinden da¬
ha sonra sırasıyla tıp fakültesi, devlet hastanesi, sağlık ocağı ve
özel kurumlar tarafından verilmişti. Antibiyotik tedavilerinin
%39,1'i üst solunum yolu enfeksiyonu tanısıyla yapılmıştı. Bunlara
otit, sinüzit, tonsillofarenjit ve alt solunum yolu infeksiyonları
eklendiğinde vakaların yaklaşık %70'ini solunum yolu enfeksiyonları,
%9'unu ürogenital sistem enfeksiyonları oluşturmaktaydı. En
sık penisilin-beta laktamaz inhibitörleri, 2. kuşak sefalosporinler,
makrolidler ve antibiyotiklerin oral formları tercih edilmişti. Antibiyotik
tercihleri kurumlara göre değişiklik göstermekteydi.
Sonuç: Çalışma antibiyotiklerin yüksek oranda kullanımını göstermektedir.
Dörtte üçünün solunum yolları enfeksiyonu için kullanılmış
olması solunum yolu enfeksiyonlarının tanı ve tedavisinin sürekli
tıp eğitiminde gözden geçirilmesi gerektiğini düşündürmek¬
tedir.
